2.1 Summary
This routine inspection marks the first round of laboratory inspections since the COVID-19 pandemic and it is also the first inspection since the establishment of department in 2022. Overall, the safety standards observed during the inspection are adequate, with evidence of various good practices as listed in Part 2.2. For instance, the labelling practice is good, and the containers with chemicals were all labelled clearly to avoid confusion, the sample boxes were arranged neatly on the bench. Additionally, appropriate containers were available to transfer liquid nitrogen whenever necessary, which can prevent potential pressure build-up inside container. In light of the aforementioned observations, it is evident that continuous and sustained efforts have been made to maintain safety standards. The Departmental Safety Representative, along with all technical staff and individuals involved, deserve recognition for their commitment and contributions.

Inevitably, there are still areas that require further attention and improvement. Some findings of particular concern have been identified. First, it is noticed that xxxxxxxxxxx xxxxxxxxxxxx xxxxxxxx xxxxxxxxxxxxxx xxxxxxxxxxxxxxxxxxxxxxxxxxx xxxxxxxxxxxxxxxxxxxxxxxxxxxx xxxxxxxxxxxxxxx . Second, the practice of recapping needles before disposal was identified. This high-risk process can lead to needlestick injuries and should be avoided. The department is advised to review the current sample handling workflow and update safe practices for users. Third, the department is suggested to review the xxxxxxxxxxxxxxxxxxxxxxxxxxxx, then proceed to prepare the necessary documents and take additional actions to fulfil the health and safety responsibilities assigned to different stakeholders.

Other minor shortcomings are listed in Part 2.3, including but not limited to absence of biohazard warning sign on freezer and/or fridge, and not-updated health and safety placard on the door of the laboratory. It is worth noting that the findings listed in this report are based on the observed findings and they are not exhaustive. Therefore, it is important not only to take the required actions but also to consider the reported findings as examples and check for similar shortcomings in all the laboratories or rooms under the purview of the department in order to reduce the risk at workplace. The required actions suggested remedial actions, but other improvements that are equivalent in reducing the risk are also acceptable. Moreover, it is crucial to communicate the risks with all laboratory users in raising their awareness of laboratory safety. With the departmental support in laboratory management and continuous education for laboratory users on good laboratory practices, these efforts promote the development of a sustainable positive safety culture within the school in the long run.
